Back in September, Mark Manicad made a big decision – to undergo PRK surgery.


	With an eye grade of 475, the actor-model known for his role as Police Captain Edwin Salonga in FPJ Ang Probinsyano booked a LASIK screening with Dr. Emerson Cruz and was qualified for PRK.


	Photorefractive Keratectomy or PRK is a type of refractive surgery done to correct refractive errors, which are more commonly known as nearsightedness, farsightedness, and astigmatism. According to Asian Eye Cornea and Refractive/LASIK Surgeon Dr. Emerson Cruz, PRK is an alternative to LASIK. “LASIK is the more popular method, but PRK is also quick and painless. The end goal for both surgeries is to help improve the patient’s vision without having to wear eyeglasses or contact lenses.”


	Often recommended to patients who have chronic dry eyes, thin corneas and high eye grade, PRK is done differently from LASIK and has longer healing time. He explained, “In LASIK, we use laser to create a corneal flap before we use another laser to reshape the cornea. We then return the flap to its original position. But in PRK, we scrape off the surface layer of the cornea using an alcohol-based solution; we then apply laser to reshape the cornea. A temporary bandage contact lens is then placed to protect the cornea while it’s healing.”


	“You may feel discomfort the first few days after the PRK surgery,” Dr. Cruz added. “Over-the-counter medicines can help relieve it, and eye lubricants are needed to keep your eyes moist. The complete healing process may take one to three months.”

Asian Eye Tele: What You Can Expect When You Consult Online

Diabetes is one of the most prevalent chronic diseases in the world. Based on the 2020 data of the International Diabetes Federation (IDF), there were over 3,993,300 cases of diabetes in adults in the Philippines in 2017.


	Diabetes results in raised blood sugar. Over time, it can lead to serious complications, such as stroke, heart attack, or kidney failure. Diabetes can also damage the eye. The most common diabetes-related eye disease is diabetic retinopathy.


	“Diabetic retinopathy happens when you have elevated and poorly controlled blood sugar,” explains Asian Eye Institute Retina and Vitreous Disease Specialist Dr. Amadeo Veloso, Jr., “The blood vessels in the retina get blocked, so the blood supply is cut off.”


	The retina is the back part of the eye that senses light and sends images to the brain. To function, it needs a continuous supply of blood. “Without the blood supply, the eye attempts to grow new blood vessels that are weak and leak easily. This is when patients start to experience eye problems,” he adds.


	Usually affecting both eyes, many patients do not know they have diabetic retinopathy until symptoms arise. These include seeing dark strings floating across vision (floaters), blurred vision, poor night vision or inability to see colors. Left untreated, it can lead to visual impairment or blindness.


	Mrs. Violeta Villareal suffered from sudden blurring of vision. “Six years ago, I felt changes in my vision. Sometimes, my vision was blurry. Other times, I saw floaters and colors seemed faded. It was really hard to do things at home, read and write, and drive, especially at night.”


	Diabetes runs in her family – from her grandparents to her siblings. She recalls, “I was 42 when I learned I was diabetic. I felt dizzy and got tired easily. I lost weight, and I felt hungry or thirsty even when I just ate. Being married to a doctor, he knew something was wrong. He then decided to take me to an executive checkup.” Mrs. Villareal was referring to her husband, Dr. Hermogenes Villareal, a renowned dentist.


	She figured she would only feel those symptoms whenever her blood sugar is up. “I felt the need to always get my eyeglass lenses changed. Then I was recommended by a good friend to Asian Eye. When I went for a checkup, I was diagnosed with diabetic retinopathy and recommended for a laser surgery.”


	Treatments available do not cure diabetic retinopathy, but they help delay its progression and prevent permanent blindness. “The treatment that will be done depends on the extent of patient’s eye condition. They may undergo laser surgery, injection therapy or vitrectomy,” says Dr. Veloso, “In Mrs. Villareal’s case, we did laser surgery to shrink abnormal new vessels.”


	“Dr. Veloso managed to protect my eyes from possible blindness,” shares Mrs. Villareal, “Can you imagine? I received 400 laser shots for each eye. But he was so good that I did not feel a thing! I even got back to doing normal activities a few days after surgery. My vision is now clearer. I only wear reading glasses.”


	Dr. Veloso emphasizes that maintaining a healthy lifestyle is the best way to keep patients from going blind and other complications of diabetes. “Prevention is better than cure, so make it a habit to control your blood sugar, blood pressure, and cholesterol levels; eat healthy; exercise regularly; and avoid smoking.”


	“I agree with Dr. Veloso,” she says, “This experience made me realize how hard it is to have an eye problem. That is why I have been keeping good control of my diabetes – I take my medicine and insulin on time, maintain a healthy diet, avoid eating too much sweets and exercise regularly. Most importantly, I see my endocrinologist and Dr. Veloso regularly. I work closely with them to make sure that my blood sugar is normal and that my eyes are healthy.”


	“A comprehensive eye exam every six months is also a must for diabetics,” he says. “It is important to check the retina of the patient, so we can determine if the patient’s vision has been affected by diabetes and what treatment can be done to prevent complications like retinal detachment, glaucoma, or blindness.”


	“Patients who are not diabetics should also get their eyes checked even before they notice any changes in their vision.”

Imagine trying to read the time but not seeing the clock’s long and short hands, browsing a book without seeing the text, or looking at your family member’s face and not recognizing them nor perceiving their expression. Straight lines look wavy or crooked, colors are faded, and objects look smaller than usual. This is what people with age-related macular degeneration usually experience.


	Age-related macular degeneration, more commonly known as AMD, is among the leading causes of blindness among senior citizens.


	According to Asian Eye Institute Retina and Vitreous Disease Specialist Dr. Patricia Quilendrino, “While it’s not painful, AMD affects a person’s macula or the central vision. The macula helps you see fine details and recognize colors. We still don’t know the exact cause of this condition, but it’s more likely to develop in women, and people who have family members who’ve had it, those who suffer from hypertension, are overweight, and are smokers.”


	Rarely causing total blindness, AMD can be classified into dry or wet categories. Dry AMD develops with age and takes time to cause vision loss. Wet AMD, on the other hand, is less common but is more serious and causes vision loss faster. She says, “It happens when abnormal blood vessels grow in the retina. And because these blood vessels are weak, they may leak blood or other fluids, leading to vision loss or distortion.”


	There is no cure yet for AMD, but there are ways to prevent vision loss. People with dry AMD may be given supplements or vitamins and be asked to eat a lot of food rich in nutrients that are good for the eye, including yellow fruits, dark, leafy greens, and fish. For those with wet AMD, they may undergo anti-vascular endothelial growth factors (anti-VEGF) treatment. Anti-VEGF are injected in the eye to effectively stop the growth of new blood vessels, bleeding and swelling. It is important to remember that when left untreated, vision may get worse.


	With the current COVID-19 pandemic, it has been hard for patients, particularly senior – age parents or grandparents to seek eye checkup and treatment. A recent study shows that delay and lapses in treatment affected the vision of patients. Dr. Quilendrino explains, “Results showed that patients who had lapses in treatment had increased thickness in the macula. Although it normalized when they started receiving treatment again, they did not recover the visual acuity they lost.”


	Visual acuity refers to the clarity or sharpness of your vision.


	“We understand that some patients are still afraid to go out, and it can be challenging especially for those that live in the provinces or those with co-morbidities. However, consistent evaluation and treatment are critical to ensure that AMD patients won’t lose their vision. We are also implementing strict COVID-19 protocols to ensure that our patients, their companions and even our team will continue to be safe and healthy.”


	Aside from the typical health screening, social distancing practices and wearing of PPEs, almost 90% of Asian Eye employees have been vaccinated. It's good to have this service in this current situation, also considering that I am from the province so I'm not able to go to Manila as of now.

I love the teleconsultation service provided by Asian Eye. The staff, Ema is very approachable and consistent in helping us set up everything. There was no hassle at all. Also, the doctor was able to examine my mom just like we are in the clinic and i dont feel like we are being rushed to finish the consultation.

My son was the patient and I really dont want to bring him to any clinic for a consultation due to the pandemic. So I decided to search for Asian Eye Institute if the have online consultation and great thing they do! It was absolutely helpful especially with our current situation. I really appreciate how Emma assisted me throughout the process. Thank you Dr. Fajardo for accommodating us. The app is really a big help for everybody.
